Foster / Adoption Intake Form

This form provides information for us regarding your availability to receive animals from The Simple Heart and other associated groups. Note that many of the animals we rescue suffer from trauma and may have behavioral issues. Others may be rescued in situations where the rescue is legally challenged; you should “know your rights” and be in contact with The Simple Heart legal team (legal@simpleheart.org) if any issues arise.

It is important that you understand and be prepared for the difficulties of taking animals from cruelty scenarios. Rescues are transformative, for both the animal rescued and for their new human family. However, everyone should understand that rescues come with many challenges.
